We are seeking a compassionate and organized Social Worker/Case Manager/SNF Resident Coordinator to join our team at Encore at Foulk!
Under the direction of the Administrator, this part‑time role supports the social, emotional, psychosocial, care coordination, and discharge planning needs of residents in a Skilled Nursing Facility. Acts as a liaison between residents, families, staff, and community resources to ensure quality, resident‑centered care. Schedule options: M,W,F or M‑F 4 Hour shifts.
Responsibilities:- Conduct psychosocial assessments upon admission and ongoing
- Provide counseling, emotional support, and crisis intervention
- Coordinate discharge planning and transitions between levels of care
- Participate in care plan meetings and interdisciplinary team (IDT) meetings
- Monitor and support resident care plans and communicate changes to staff
- Coordinate services and community resources to meet resident needs
- Assist with admissions and internal room transfers as needed
- Document assessments and interventions in the EMR per regulations
- Support quality improvement and regulatory compliance initiatives
- Degree in Social Work or related field preferred
- Healthcare or long‑term care experience preferred
- Strong communication and documentation skills
- Ability to work collaboratively with residents, families, and staff
