Speech Language Pathologist

Encore at Foulk is hiring a Speech Language Pathologist for both Skilled and Outpatient Departments.

$2,500 sign-on bonus!!

The $2,500 sign-on bonus will be paid in installments. Continued employment with the company for a minimum of one (1) year from the start date is required. If employment ends prior to the one-year anniversary, any unpaid portion of the bonus will be forfeited.

The Speech Language Pathologist (SLP) organizes and provides speech-language services for the facility to facilitate rehabilitation. They direct patient participation in selected tasks to restore, reinforce, and enhance performance. Also, the Speech Language Pathologist facilitates the learning of those skills and functions essential for communication and swallowing, to diminish or remediate disorders or deficits. They direct activities to promote and maintain health, within the scope of practice and consistent with the Code of Ethics of the profession.

• Evaluate, diagnose, and develop individualized treatment plans for speech, language, cognitive, and swallowing disorders.
• Provide skilled therapy interventions, perform re-evaluations, and maintain accurate, timely documentation.
• Collaborate with the interdisciplinary team, participate in care conferences, and make appropriate referrals as needed.
• Educate patients and families, coordinate adaptive communication devices, and ensure patient safety and equipment maintenance.
• Supervise CFYs and support staff as applicable and perform FEES if qualified and trained.

• Graduate of an accredited Speech Language Pathologist (SLP) program.
• Current DE state licensure.
• CPR Certified.
