This is a contract on-site role for a Care Manager, Registered Nurse located in Wilmington, DE. The Care Manager will coordinate and oversee patient care, manage nursing services, and act as a key liaison for ensuring high-quality healthcare. Day-to-day tasks include assessing patient needs, collaborating with the interdisciplinary healthcare team, and implementing care plans to optimize patient outcomes. The role requires delivering compassionate care and managing critical and complex patient situations effectively.
Qualifications- Strong expertise in Nursing, Patient Care, and Critical Care Medicine
- Experience with Nursing Management and providing leadership in medical care settings
- Knowledge of Medicine and medical best practices
- Effective communication, teamwork, and patient advocacy skills
- Current and valid Registered Nurse (RN) license in Delaware or compact states
- Certified Case Manager (CCM) certification is a plus
- Experience with care coordination or case management in a clinical setting
