LPN I - Outpatient - Access Center - M-F, 7:30a-4p
PRIMARY FUNCTION:
The Licensed Practical Nurse (LPN) – Medication Refill is responsible for the review, processing, and coordination of prescription refill requests in accordance with established clinical protocols, state regulations, and organizational policies.
This role serves as a centralized clinical resource supporting safe medication management by collaborating with providers, pharmacists, and care teams to ensure accurate, timely, and compliant refill processing.
The LPN functions within scope of practice, utilizes clinical judgment, and escalates concerns requiring provider intervention.
PRINCIPAL DUTIES AND RESPONSIBILITIES:
• Reviews and processes incoming prescription refill requests from patient phone calls, patient portal messages, pharmacies, and electronic refill queues.
• Verifies patient demographics, medication history, allergies, laboratory requirements, and last visit information prior to refill processing.
• Assesses refill eligibility based on established clinical protocols, standing orders, and provider-approved guidelines.
• Identifies medication discrepancies, potential drug interactions, contraindications, or required monitoring labs and escalates to the provider as appropriate.
• Ensures compliance with state regulations, organizational policies, and controlled substance guidelines.
• Documents all patient interactions and refill actions accurately in the electronic health record (EHR).
• Communicates clearly with patients regarding refill status, required follow-up appointments, laboratory monitoring, or provider review.
• Collaborates with providers and pharmacists to resolve refill issues or clarification requests.
• Monitors assigned refill work queues to maintain productivity and service level standards.
• Adheres to departmental quality, safety, and productivity metrics.
• Maintains patient confidentiality in accordance with HIPAA regulations.
• Performs other related duties as assigned.
EDUCATION AND EXPERIENCE REQUIREMENTS:
Graduate of an accredited school of practical nursing required.
2 years’ experience as an LPN required
KNOWLEDGE, SKILL, AND ABILITY REQUIREMENTS:
• Knowledge of pharmacology principles, medication safety standards, and refill best practices.
• Ability to interpret laboratory values relevant to medication management.
• Understanding of chronic disease management and medication monitoring requirements.
• Proficiency in electronic health record documentation.
• Strong written and verbal communication skills.
• Ability to multitask while reviewing clinical information and communicating with patients.
• Demonstrates sound clinical judgment and escalation awareness.
• Maintains compliance with infection control, safety, and confidentiality standards.
QUALITY AND COMPLIANCE EXPECTATIONS:
• Meets established turnaround time benchmarks for refill processing.
• Maintains accuracy and documentation standards consistent with audit and regulatory requirements.
• Participates in quality improvement initiatives related to medication safety and workflows
SPECIAL REQUIREMENTS:
Current LPN State licensure and CPR certification.
Hourly Pay Range: $21.03 - $31.54This pay rate/range represents Christiana Care’s good faith and reasonable estimate of compensation at the time of posting. The actual salary within this range offered to a successful candidate will depend on individual factors including without limitation skills, relevant experience, and qualifications as they relate to specific job requirements.
