Nationwide Healthcare Services is looking for clinicians to join our rehabilitation departments in the Wilmington and Hockessin Delaware areas.
We have Full-Time, Part-Time and PRN positions available.
RESPONSIBILITIES Therapist Specific- Evaluates residents and develops appropriate care plans.
- Monitor residents' progress and adjust treatments accordingly.
- Under a physician’s order, provides direct resident treatment in accordance with an established plan of care. Treatment modalities may include; individual, concurrent and group.
- Provide excellent training for residents and caregivers.
- Documents services provided within Rehab Optima on a timely basis.
- Demonstrates consistent knowledge and application of Medicare and other reimbursement regulations.
- Exhibits courtesy, compassion and respect to residents, families, caregivers, visitors, physicians, administrators, and co-workers in each and every interaction.
- Demonstrates continuous new learning to remain current in the field.
- Complies with facility Privacy Practices policies and procedures to protect residents’ individual health information.
- Participates in clinical program development and implementation.
- Contributes to the team meeting and decision-making process as defined by the program.
- Responsible for service and operational excellence of all assigned activities to ensure the delivery of quality outcomes.
- Participates in the training and orientation of other staff, students and volunteers.
- Participates in quality improvement activities as needed.
- May perform additional assigned administrative duties like: assigning and overseeing resident care activities, medical record oversight, assuring execution of physician orders, ordering supplies, etc.
- Graduated from an accredited program approved by their professional association.
- Current licensure by the applicable state agency.
- Clinical experience in a skilled nursing facility is preferred.
