Nationwide Healthcare Services is looking for clinicians to join our rehabilitation departments in the Wilmington and Hockessin Delaware areas.

We have

Full-Time, Part-Time and PRN positions available.

RESPONSIBILITIES Therapist Specific
  • Evaluates residents and develops appropriate care plans.
  • Monitor residents' progress and adjust treatments accordingly.
  • Provide supervision for Occupational Therapy/Physical Therapist Assistants, students and support personnel in accordance with state license requirements and professional standards
All Clinicians
  • Under a physician’s order, provides direct resident treatment in accordance with an established plan of care. Treatment modalities may include; individual, concurrent and group.
  • Provide excellent training for residents and caregivers.
  • Documents services provided within Rehab Optima on a timely basis.
  • Demonstrates consistent knowledge and application of Medicare and other reimbursement regulations.
  • Exhibits courtesy, compassion and respect to residents, families, caregivers, visitors, physicians, administrators, and co-workers in each and every interaction.
  • Demonstrates continuous new learning to remain current in the field.
  • Complies with facility Privacy Practices policies and procedures to protect residents’ individual health information.
  • Participates in clinical program development and implementation.
  • Contributes to the team meeting and decision-making process as defined by the program.
  • Responsible for service and operational excellence of all assigned activities to ensure the delivery of quality outcomes.
  • Participates in the training and orientation of other staff, students and volunteers.

    Participates in quality improvement activities as needed.
  • May perform additional assigned administrative duties like: assigning and overseeing resident care activities, medical record oversight, assuring execution of physician orders, ordering supplies, etc.
EDUCATIONAL REQUIREMENTS:
  • Graduated from an accredited program approved by their professional association.
  • Current licensure by the applicable state agency.
EXPERIENCE REQUIRED
  • Clinical experience in a skilled nursing facility is preferred.
