General Summary:
The Population Health Pharmacist is a licensed pharmacist providing clinical pharmacy support to Medicare and Commercial providers within value-based care arrangements ponsibilities focus on improving quality of pharmacy-related care (STAR measures) and cost containment strategies for Delaware residents.

Responsibilities:

  • Analyze reports and EMR data to identify opportunities for improving outcomes (STAR/HEDIS) and reducing pharmacy costs.

  • Assess health plan members with pre-determined conditions, identify barriers to care, provide self-management education, recommend screenings, perform MTM services, and develop personalized health improvement plans.

  • Collaborate with providers on clinical quality data initiatives; maintain a 5-STAR rating of 90% or greater annually.

  • Participate in committees, meetings, and audits; develop and maintain policies and procedures.

  • Educate providers and members on formulary changes annually.

  • Identify member needs and intervene to prevent avoidable hospital admissions/readmissions; document interventions.

  • Act as subject matter expert in pharmacy-related clinical measures for value-based care agreements.

  • Other duties as assigned.

    Requirements

    Required Education, Credentials, and Experience:

    • Education: Bachelor's Degree in Pharmacy

    • Credentials: Licensed Pharmacist; BPS Certified in a specialty (Pharmacotherapy, Ambulatory Care, Cardiology, Geriatrics, or related area)

    • Experience: One year of hospital pharmacist experience as a clinical pharmacist OR completion of ASHP accredited PGY-1 residency progra

    Preferred Education, Credentials, and Experience:

    • Education: Doctorate Degree in Pharmacy (PharmD)

    • Credentials: Board Certification with one year of clinical pharmacy experience OR PharmD with Board Certification in Pharmacotherapy OR advanced degree (e.g., MPH or MBA)

    • Experience: Two years of hospital pharmacist experience as a clinical pharmacist OR completion of ASHP accredited PGY-2 residency ambulatory program
