What Youx
27ll Do

  • Build trusting relationships with patients through outreach, individualized support, and ongoing follow-up.
  • Help patients identify health goals and overcome social, economic, and other barriers that may affect their health and access to care.
  • Connect patients with healthcare services, community programs, and social resources while helping them navigate the healthcare system.
  • Partner with healthcare teams and community organizations to coordinate patient-centered support and advocate for patients' needs.
  • Engage with patients in their homes, Christiana Care facilities, and community settings while managing an assigned caseload and documenting patient progress.

Qualifications

  • High school diploma or GED.
  • 1-2 years of experience in community outreach, healthcare support, social services, or a related field.
  • Strong communication, relationship-building, organizational, and problem-solving skills.
  • Ability to work independently and collaboratively with patients, healthcare professionals, and community partners.
  • Valid driver's license and access to a reliable personal vehicle.

Preferred Qualifications

  • Associate or bachelor's degree in Public Health, Social Work, Health Education, or a related field.
  • Community Health Worker Certification.
  • Experience working with underserved or diverse populations.
  • Bilingual communication skills.

Work Schedule & Location

This is a full-time, community-based position in New Castle County with occasional travel to Kent/Sussex Counties
. You'll work with patients in their homes, community locations, and Christiana Care clinical settings. Flexibility to work occasional evenings and weekends is required.
