RN Case Manager

As compassionate professionals, the RN Case Manager develops and oversees the plan of care for patients dealing with a serious, life-limiting illness, working closely with the patient’s provider and the hospice interdisciplinary team. To promote comfort and quality of life the RN provides skilled nursing care; guidance and education to families and caregivers on recognizing and managing potential symptoms; what to expect;

while providing emotional and practical support for both the patient and the family/caregiver. Helios Care RN Case Managers have one-on-one time providing exceptional care and attention to the patients, brightening their days and making a lasting positive impact.

Full-time

Pay range: $77,500 – $80,000
