Telehealth Coordinator/Chronic Care Registered Nurse Position SummaryThe RN Telehealth Coordinator helps facilitate virtual medical consultations and appointments for patients who require specialized care beyond the facility's capabilities. The position monitors patients from intake through release into the community, ensuring continuity of care and acting as a liaison among multiple systems to support offender health.
The chronic care nurse monitors patients from intake to release in the facility, ensuring continuity of care and advocating for patient needs.
Minimum Requirements- Graduate of an accredited School of Nursing
- Active licensure as a Registered Nurse in the state of employment (Delaware or multistate)
- Active CPR certification
- Knowledge of specific state laws and regulations governing practice
- Completion of initial and annual clinical competencies
- Maintain accurate, detailed reports and records; keep up-to-date medical records for patients receiving specialized care, documenting histories, test results, treatment plans, and communications.
- Administer medications and monitor patients for reactions and side effects.
- Monitor, record, and report symptoms or changes in patients’ condition.
- Coordinate with outside healthcare providers, schedule appointments, and prepare necessary medical records.
- Perform administrative tasks such as scheduling appointments and maintaining records.
- Additional duties as assigned by facility.
