• Administer medications orally, topically, IV, injectable.
• Perform wound care, prevention, manage pain, apply dressings.
• Monitor physical and mental health status, track changes and analyze data.
• Provide catheter/ostomy care, gather specimens.
• Assist in executing individualized care plans.
• Collaborate with physicians, therapists, social workers and others.
• Educate families and residents on health status, changes, and expectations.
• Document health status, all care, progress and interactions.
• Oversee and delegate CNAs ensuring quality care and teamwork.

• Licensed in the State of Delaware as an LPN.
• Pass background check, drug screen, professional references and physical evaluation.
• Professional, clinically skilled, with leadership qualities.
