Saint Francis Hospital, a member of Trinity Health Mid-Atlantic, is seeking an experienced Registered Nurse to join the wound care team.

Schedule:

Part-time, Monday, Wednesday, and Friday, 8am–4pm.

Responsibilities
  • Conduct and document detailed physical exams and evaluate and manage patients with wounds and extensive case management.
  • Perform case management functions including assessing and providing for wound care needs post-discharge; collaborate with providers and other medical personnel; order equipment and supplies.
  • Work with the wound care team including physicians, physical therapists, and other nurses.
  • Educate patients and families about chronic wounds and maintain appropriate follow-up care.
  • Manage in-patients as well as out-patients with complex wounds; treat patients through clinical pathways; follow standards of care and advanced clinical practice.
  • Educate and support in-patient nurses on maintenance of skin integrity, prevention and treatment of skin breakdown and wounds.
  • Adhere to hospital nursing practice standards; perform history and physical assessment, planning, implementation, and evaluation of patient care.
  • Implement appropriate nursing actions for each patient; perform thorough history and physicals on every patient in the wound center.
  • Make decisions reflecting knowledge and sound judgment; discuss with patient and/or family about planned care and outcomes and document it.
  • Identify and utilize a multidisciplinary approach to deliver patient care; evaluate patient and family understanding of instructions; provide written instructions and materials.
  • Review preventative health measures with patient and family; document understanding of instructions; complete all charts and forms accurately and timely.
  • Transmit knowledgeable and concise patient reports as needed to demonstrate awareness of the patient’s overall condition.
  • Initiate Clinical Pathways/Plan of Care for appropriate patient populations at first visit; update Care Plans and Pathways as needs change.
Minimum Qualifications
  • Current licensure as a Registered Nurse in the State of Delaware required.
  • ASN required; BSN preferred, from an accredited school of nursing.
  • At least 3 years of clinical experience required, preferably in Wound Care.
  • Wound Care certification preferred; will consider those with experience and able to obtain certification within 6 months of hire.
