Job Responsibilities
- Oversee a clinical multidisciplinary team, planning, controlling, organizing, and leading to provide effective and efficient clinical services.
- Supervise, support, and educate staff on site in the field.
- Support an efficient and accurate revenue cycle by enhancing appropriate revenue capture.
- Manage an active patient census as determined by the Director of Operations, assigning care, maintaining clinical integrity, and overseeing the flow of documentation.
- Mentor team competency in clinical assessment, OASIS gathering and interpretation, time management, point‑of‑care documentation, and operational support systems.
- Build and evaluate team members through hiring, performance appraisals, and holding clinicians accountable to agency standards.
- Collaborate with the Workflow Clinical Manager, PI, and Education Coordinators on OASIS integrity and documentation.
- Directly supervise RNs, LPNs, PTs, PTAs, OTs, OTAs, SLPs, MSWs, HHAs, Team Assistants and provide indirect supervision of the Medical Records Specialist.
- Ensure clinicians complete mandatory orientation, education, and weekly team meetings; conduct supervisory field visits and maintain compliance documentation.
- Participate in agency initiatives, committees, and quality improvement projects to advance program effectiveness.
- Maintain timely submission of all clinical documentation and performance reviews; respond to internal and external communications within two hours.
- Create a cohesive team focused on customer satisfaction, confidentiality, ethical practice, and continuous improvement.
- Education: Graduate from an accredited nursing or physical therapy school; bachelor’s degree preferred.
- Licensure / Certification: Current license to practice professional nursing or physical therapy in the State where staff is supervised (NC/SC).
- Experience: Minimum two years of clinical practice; minimum one year of home health experience preferred; prior clinical, teaching, and supervisory experience within a nursing service organization preferred.

Current knowledge of medical‑surgical and home health nursing principles, adult learning principles, standards of medical record keeping; ability to communicate clearly in oral and written form and be literate in English. - Interpersonal
Skills:
Excellent communication skills, professional and tactful interaction with coworkers, physicians, patients, families, and the public; flexibility and innovation. - Physical Requirements: Ability to stand or sit for extended periods; ability to stoop, kneel, bend, push, reach; ability to supervise or precept staff during patient care activities.
- Mental Requirements: Ability to coordinate agency resources, facilitate teamwork, demonstrate strong problem‑solving and reasoning, prioritize duties, learn new skills, work independently.
- Sensory Requirements: Ability to use sight to assess, plan, implement, and evaluate patient care activities; hearing to communicate.
- Exposure: Noise, office equipment, machinery; long periods computer and chart review; travel required; temperature changes or weather extremes.
- Hours of Work: Monday‑Friday with weekends and holidays as needed; flexible hours, on‑call rotation, and ability to respond as needed.
- Transportation: Valid North Carolina driver’s license and operational vehicle.
