Key Responsibilities
  • Assess and monitor patients’ health conditions in their home environment.
  • Develop and implement individualized care plans.
  • Educate patients and families on health management and self‑care.
  • Collaborate with a multidisciplinary team to ensure optimal patient outcomes.
  • Apply the nursing process (assess, plan, implement, evaluate) for all patients across developmental stages: newborn, infant, child, adolescent, adult and geriatric.

Primary

Job Duties
  • Assesses, interprets, plans, implements and evaluates patients according to the patient’s age and diagnosis.
  • Effectively and efficiently manages the care of a caseload of patients and coordinates care with a multidisciplinary team.
  • Appropriately communicates information in accordance with agency policies and procedures and discipline–specific guidelines.
  • Contributes to program effectiveness.
  • Organizes and performs work effectively and efficiently.
  • Maintains and adjusts schedule to enhance agency performance.
  • Demonstrates a daily commitment to the values of the agency.
  • Demonstrates positive interpersonal relations in dealing with all members of the agency.
  • Maintains and promotes customer satisfaction.
  • Effectively demonstrates the mission, vision, and values of the agency on a daily basis.
  • Job Specifications
  • Education:

    Graduate of an accredited or approved school of nursing, either an AD, Diploma, or BSN program.
  • Licensure /

    Certifications:

    Current license to practice professional nursing in the state in which care is provided (NC/SC). CPR certification required.
  • Experience:

    One year RN experience and a total of two or more years clinical experience is required. Home‑health experience preferred. Supplemental experience may include LPN, CNA, military medic, EMT or related experience.
  • Essential Technical / Motor

    Skills:

    Hand/eye coordination for injections, computer use, and patient equipment handling; proficiency in English; ability to manipulate and transfer patients.
  • Interpersonal

    Skills:

    Ability to develop positive interaction with patients, families, physicians and staff.
  • Essential

    Physical Requirements:

    Ability to transfer or maneuver objects weighing at least 50 pounds; frequent pushing, moving, lifting of patients; occasional overhead reaching, stair climbing and fine‑motor tasks.
  • Essential Mental Abilities:
    Ability to assess, plan, select interventions, evaluate responses, explain and teach patients; higher level mental faculties, short‑ and long‑term memory, prioritize duties, learn new skills.
  • Essential Sensory Requirements:
    Visual assessment for dressing changes, IV insertion; hearing for auscultation and alarms; effective communication.
  • Exposure to Hazards:
    Noise, blood‑borne pathogens, infectious diseases, needle puncture wounds, animal and traffic hazards, temperature extremes.
  • Hours of Work:
    Variable weekdays, weekends, holidays as needed; flexible schedule to accommodate staffing needs.
  • Population Served:
    Adolescents, adults, geriatrics and children.
  • Driver’s License:
    Must have a valid North Carolina driver’s license and an operational vehicle.
