As a Nurse Liaison for Christiana Care Home Health you will identify home care needs of patients to ensure a smooth transition from acute care and post acute care settings; ensuring the patient meets home care requirements, verify insurance, confirm patient demographics. The Nurse Liaison supports the intake process by interacting with discharge planners, case managers and patients within the acute and post acute care settings.

The successful candidate will:

• Work up to 20 hours per week; the hired individual will work 8 hours on Saturday and Sunday and an additional 8 hours during the week which can be flexible. (Must work 40 hours over two weeks including all Saturdays and Sundays)
• Flexibility to work remote, onsite at our Smyrna office. Orientation is in person on the weekends at Smyrna or during the week at Ave North.
• Commitment to work 2 holidays a year required; one winter and one summer.

Requirements:

• LPN licensed to work within Delaware
• Previous experience with Home Health is preferred
• Strong knowledge of Medicare, Medicaid, and third-party payer requirements/regulations.
• Understanding of Epic, teams, outlook is helpful.
• Strong team player skilled at communication required.
• Sound clinical judgement

Hourly Pay Range: $27.31 - $40.96

This pay rate/range represents Christiana Care's good faith and reasonable estimate of compensation at the time of posting. The actual salary within this range offered to a successful candidate will depend on individual factors including without limitation skills, relevant experience, and qualifications as they relate to specific job requirements.
