Community-Based Child Behavioral Health Case Manager

We are seeking a Case Manager to join our Children’s Behavioral Health Program serving clients in New Castle County, Delaware.

• At least 1 year of experience working with children and adolescents
• Bachelor’s degree
• Valid driver’s license and clean driving record
• Available to work Monday‑Friday with an on‑call rotation

• Coordinate resources and information pertinent to client needs.
• Document all coordination of resources efficiently and accurately.
• Coordinate referrals for client‑specific discharge planning and ensure the family actively participates in a multi‑faceted discharge plan.
• Monitor record‑keeping compliance and ensure client charts are up‑to‑date, including documentation of significant incidents within 24 hours.
• Monitor and assess document safety.
• Complete necessary documentation of each case to comply with standards set by Merakey, county MHDS/CASSP, OMHSAS, and other licensing/regulatory/payor organizations.
• Conduct case sessions primarily in the community (client homes, schools, etc.). Provide case management in an office environment or community setting and be willing to work evenings and/or weekends; may require on‑call coverage.
