Case Manager – Children’s Behavioral Health Program
We are seeking a Case Manager to join our Children’s Behavioral Health Program serving clients in New Castle, Delaware.
- Coordinate resources and information pertinent to client needs.
- Effectively and efficiently document all coordination of resources.
- Coordinate referrals for client-specific discharge planning; ensure that the family actively participates in creating a multi-faceted discharge plan that reflects the family’s choice of service providers and community resources.
- Monitor record keeping, compliance, and authorized units remaining, ensuring that client charts are up‑to‑date and include all necessary documentation, including documentation of significant incidents within 24 hours as needed (incident reports).
- Monitor/assess document safety.
- Complete necessary documentation of each case, complying with the standards set by Merakey, county MHDS/CASSP, OMHSAS, and other licensing, regulatory, and payor organizations.
Earn $20.67/hour

Requirements:
- Bachelor’s degree in a related field.
- Experience in case management or related areas.
- Valid driver’s license with reliable transportation is required.
