The incumbent is responsible for providing assessment, case management of specialty court clients referred by Delaware Courts. Clients are in the community & under DOC supervision. Work includes: conducting assessment of behavioral health needs, referring clients to appropriate care, following client progress, reporting to court via status hearings & reports, assisting clients with accessing medical care, housing, education and other services, conducting urine drug screen monitoring.
