Overview
Horizon House is a nationally recognized provider of services to adults with support needs due to developmental disabilities, behavioral health issues and substance abuse. Horizon House is Pennsylvania’s oldest psychiatric rehabilitation agency. Since 1952, Horizon House has been dedicated to helping people with mental illness, drug and alcohol addictions, developmental/intellectual/disabilities and homelessness to help stabilize their lives and to help them live as independently as possible in the community.
With over 100 locations throughout southeastern Pennsylvania and the state of Delaware, Horizon House provides services to a diverse population focusing on individual strength and choice.
We are seeking a qualified Licensed Practical Nurse (LPN) to join our Residential Group Home team at our New Castle County, Delaware location.
Work ScheduleMonday to Friday, 3P-11P
Hire RequirementsNew hire orientation first four (4) day workweek dayshift Mon‑Thu schedule at our Philadelphia location and mileage reimbursement.
Responsibilities- Assist in the development, implementation, evaluation and revision of recovery plans in collaboration with the participants and assigned staff.
- Maintain a case load of assigned participants and complete assigned paperwork in a timely manner.
- Act as a liaison consulting with community agencies and families to maintain coordination of care in the treatment process.
- Provide ongoing assessment of participants’ mental health symptoms and respond to recovery plans.
- Educate and supervise participants in self‑administration of medication.
- Ensure annual review of the participants’ physical condition and assist access to and encourage compliance with treatment for identified physical problems.
- Board Certified Licensed LPN in the state of Delaware or participating in the Nursing Licensure Compact to include the state of DE.
- Knowledge of mental health systems and supports.
- Knowledge of substance abuse systems and supports.
- Good communication skills both oral and written.
- Ability to work independently.
- Possess a valid driver’s license with an acceptable driving record.
- Strong organizational skills.
- Successful completion of the Child Abuse Registry and the Adult Abuse Registry background check with no adverse registry findings, and obtain service letters in accordance with 19
Del Code Section 708 and 11 Del Code Sections 8563 and 8564.
- Frequent talking and listening.
- Requires the ability to lift, carry, hold, kneel, stack, stoop, reach and bend which may include direct intervention with participants.
- Local travel required, including transporting participants.
- May be required to report to work during emergencies, including inclement weather.
- May require crisis intervention in emergency situations.
- Scheduling flexibility and possible on‑call work are necessary.
