POSITION SUMMARY:

Help people build healthier habits, one step at a time. As a Lifestyle Coach with the YMCA of southeastern North Carolina, you will guide participants through the YMCA’s Diabetes Prevention Program in Spanish, English, or both. You will lead group sessions, encourage participants as they work toward their goals, and deliver the program’s curriculum. You may also have the opportunity to train in other community health programs, including Blood Pressure Self-Monitoring and Healthy Weight & Your Child.

About the program:
This position is supported by a grant with a current period ending August 31, 2027
. Funding beyond that date has not been confirmed. Continued employment depends on the availability of program funding.

ESSENTIAL FUNCTIONS:

Deliver the program curriculum at YMCA and community locations using effective facilitation techniques, including open-ended questions and group discussion.

Create a welcoming, judgment-free, and noncompetitive environment. Build relationships among participants, encourage shared learning, and support participant goal setting and continued participation.

Prepare for each session by reviewing curriculum, participant notes, and food and activity trackers; developing a facilitation outline; and sending participant reminders.

Weigh participants at each session and record attendance, weight, physical activity minutes, and food tracker completion in the electronic database within required time frames.

Answer participant questions before and after sessions, provide follow-up outside of class as needed, and schedule and conduct in-person make-up sessions.

Protect participant health information in accordance with YMCA policies and procedures.

Participate in quality assurance activities, required team discussions, and professional development to maintain curriculum fidelity and meet program standards. Share facilitation ideas and tools with the team.

Provide regular updates to the Program Manager on program status and participant progress.

Report facility concerns and potential risks to the Community Health Programs Coordinator. Complete required incident and accident documentation.
