Position Summary

The Clinical Director (LPN) is responsible for ensuring the overall health, safety, and well‑being of geriatric residents through the delivery of high‑quality clinical services consistent with the assisted living philosophy. This role establishes clinical procedures, objectives, and work standards, and provides leadership, supervision, and management of direct care staff and nursing services. The Clinical Director (LPN) is accountable for maintaining a comprehensive “system of care” that promotes dignity, respect, and recognition of each resident’s individuality.

Key Responsibilities Resident Care & Clinical Oversight
  • Provide care management services, including completion of pre‑admission assessments, reassessments, and negotiated service plans.
  • Evaluate and assist in developing individualized resident care plans.
  • Coordinate admissions and orient new residents to the community.
  • Monitor residents’ health and mental status; promptly report changes in condition to physicians.
  • Review and monitor medication regimens and resident responses; make recommendations as appropriate.
  • Ensure proper completion and oversight of MARs, physician orders, narcotic logs, and special healthcare documentation.
  • Collaborate with pharmacy consultants to ensure proper handling, dispensing, and destruction of medications.
  • Observe and assist with direct resident care when necessary.
  • Supervise all outside private duty and home health nursing services.
Quality Assurance & Compliance
  • Ensure compliance with all federal, state, and local regulations.
  • Develop and implement corrective action plans for any cited deficiencies.
  • Develop and implement nursing protocols and systems to improve quality of care through QA initiatives.
  • Investigate and document resident and employee incidents; report findings to the Administrator.
  • Develop systems to ensure resident rights and requests are honored.
Safety & Environment
  • Identify safety hazards and implement timely corrective action.
  • Maintain an environment that ensures the physical and emotional well‑being of residents and staff.
  • Coordinate with the dietary department to ensure appropriate meals are served.
Leadership & Administration
  • Supervise and manage the CAL nursing department staff.
  • Direct departmental communication through staff meetings, coaching sessions, performance evaluations, and disciplinary processes.
  • Ensure completion of required staff continuing education in compliance with regulations.
  • Coordinate residency terminations, discharges, and transfers with Social Services.
  • Develop and manage the CAL nursing budget.
  • Ensure accurate and timely payroll processing.
  • Serve as liaison among residents, families, physicians, and the community.
  • Perform additional duties as assigned.
Knowledge, Skills & Working Conditions
  • Thorough knowledge of assisted living regulations and licensing requirements.
  • Proficient in computer systems and office software.
  • Ability to operate standard office and nursing equipment.
  • Ability to work in a well‑lit, well‑ventilated office and resident care environment.
  • Physical exertion required up to 40% of the time, including standing, walking, bending, and lifting.
  • Occasional lifting/carrying of objects up to 15 pounds.
  • Ability to manage emotionally sensitive situations.
  • Moderate exposure to infectious diseases and age‑related health conditions.
Education

Minimum Qualifications
  • Successful completion of a Practical Nursing program from an accredited school of nursing.
Licensure/Certification
  • Current and active LPN licensure with the North Carolina State Board of Nursing.
Experience
  • Minimum of three (3) years of assisted living experience required.
