Referral & Patient Care Coordination
- Manage incoming referrals and initiate patient contact promptly
- Educate patients clearly about services and first-visit expectations
- Remove scheduling barriers and problem-solve obstacles to care
- Ensure accurate data entry and documentation
- Track referral pipeline weekly
- Report on:
- Referrals received
- Referrals scheduled
- Evaluation arrival rate
- Identify cancellation trends and recommend solutions
- Drive goals of:
- 88%+ referral conversion rate
- 92%+ evaluation arrival rate
- Strategically fill provider schedules
- Monitor Plan of Care dashboards to support patient adherence
- Assist with schedule adjustments when providers are out
- Collaborate across clinics to ensure smooth operations
- Develop working knowledge of PT/OT specialty services
- Understand basic insurance workflows
- Communicate consistently and professionally
- Participate in ongoing training and staff meetings
- 2+ years experience in healthcare scheduling, referral coordination, or medical front office preferred
- Strong attention to detail and organization
- Clear and professional communication skills (phone and in person)
- Comfortable tracking and reporting performance metrics
- Proactive problem-solver
- Adaptable with software systems and EMR platforms
- Team-oriented with a positive attitude
32–40 hours per week
In‑clinic role with potential remote coordination tasks
