We are currently seeking an experienced:

CENTRAL INTAKE SCHEDULER

Full Time

JOB SUMMARY:

  • Coordinate and schedule all admission and routine visits for hospice, home health, and palliative patients, with priority given to urgent and same-day requests.
  • Maintain proactive and ongoing communication with referral sources, marketing team, field clinicians, intake staff, and patient families to ensure clarity, consistency, and timely follow-up.
  • Accurately input and update all scheduling activities within the EMR to support data integrity and meet regulatory standards.
  • Anticipate, identify, and resolve scheduling conflicts or delays in the admission process to prevent service disruptions.
  • Collaborate closely with interdisciplinary teams to support smooth transitions of care and ensure appropriate staff assignments.
  • Maintain clear, organized documentation of all scheduled admissions and completed visits for tracking and reporting purposes.
  • Perform additional duties as assigned to support departmental goals and patient care outcomes.

JOB REQUIREMENTS:

  • High school diploma or equivalent required
  • Associate degree or healthcare-related coursework preferred
  • Experience in Microsoft 365, medical scheduling, admissions coordination, or hospice/home health preferred. Knowledge of healthcare terminology and processes.
  • Strong organizational and time management skills, with the ability to prioritize tasks, manage multiple schedules, and meet tight deadlines.
  • Detail Oriented with a high level of accuracy.
  • Problem-solving skills to anticipate and address scheduling conflicts and delays
