Job Overview:
We are seeking an experienced Nurse Practitioner or Physician Assistant to provide comprehensive primary care services in a collaborative outpatient setting. This position focuses on preventive care, acute and chronic disease management, patient education, and community-based healthcare. The provider will work closely with physicians and interdisciplinary teams to deliver high-quality, patient-centered care across multiple clinic locations.
Key Responsibilities:
- Perform comprehensive patient evaluations, examinations, and treatment planning
- Diagnose and manage acute and chronic medical conditions
- Order, interpret, and review laboratory, imaging, and diagnostic studies
- Coordinate referrals and collaborate with specialists as needed
- Maintain accurate and timely EMR documentation
- Review laboratory, radiology, and emergency department reports daily
- Collaborate with physicians, nurses, and clinical support staff
- Participate in preventive health screenings and community outreach initiatives
- Assist with development of clinical protocols and standardized care processes
- Participate in occasional on-call coverage and travel between clinic locations as needed
Qualifications:
- Active North Carolina Nurse Practitioner or Physician Assistant license required
- National NP or PA certification required
- 1-3 years of experience in Family Practice, Internal Medicine, Urgent Care, or Community Health
- Strong clinical assessment, diagnostic, and patient management skills
- Excellent communication and documentation abilities
- Ability to work independently and collaboratively within a multidisciplinary team
- Commitment to preventive, patient-centered, and community-based healthcare
