Associate’s in Nursing or a High School Diploma (Required).
Certifications- Valid driver’s license – USA.
- Licensed Practical Nurse (LPN) – State Board of Nursing.
Experience working with adults with a mental health diagnosis and/or substance use disorder – Minimum 1 year (Not required).
Job Description SummaryThe ACTT Licensed Practical Nurse provides nursing services, case management, and supportive counseling to individuals served by the Assertive Community Treatment Team under the supervision of a registered nurse. The role includes medication management, health assessments, interdisciplinary communication, and support for independent living and community integration.
Responsibilities- Deliver nursing services, case management services, and supportive counseling under the supervision of a registered nurse to individuals served by the Assertive Community Treatment Team.
- Provide medication monitoring, medication administration, and medication education.
- Review laboratory values and report findings to the team psychiatrist.
- Evaluate physical health and make appropriate referrals to community physicians.
- Order and monitor medical supplies.
- Assess potential blood borne pathogen exposure incidents.
- Link individuals with the Assertive Community Treatment Team psychiatrist.
- Facilitate training of independent living skills, establish access to community services and agencies, assist individuals in establishing support networks in the community, and assess individual needs on a daily basis.
- Coordinate services with program staff, inpatient facilities, detoxification facilities, and establish linkage to residential programs, self‑help programs, and community resources.
- Review treatment progress and provide continuous consultation with other team staff and community agencies to ensure consistent therapeutic management of individual needs.
- Act as liaison with program after‑hours staff by exchanging participant information, detailing the presenting problem and after‑hours plans for individuals served by the team.
- Meet regularly with other team members to formulate and disseminate information, and be available by telephone and/or pager.
- Complete an intake assessment including medical history, general physical condition, alcohol and drug histories, current use, educational issues, family structure, vocational history and goals, legal history, and psychiatric history (including onset, course, effect of illness, treatment history, medication history, risk behaviors).
- Ensure immediate and appropriate interventions are provided in response to changes in mental status that may put the individual at risk.
- Document participant activities consistent with the service records manual; write event notes that reflect participant activities, goals, interventions, and outcomes.
- Develop service plans and/or person‑centered plans as appropriate, and screen referrals in coordination with other team members.
- Meet regularly to review the total caseload, problem‑solve, develop plans, and determine daily duty assignments.
- Attend and actively participate in meetings and training as required; maintain certification in all agency, state, and federal training requirements.
- Demonstrate knowledge of and comply with all agency policies and procedures, as well as state and federal statutes and regulations related to specific program areas.
- Follow service definition guidelines for services being provided.
- Driving and travel may be required.
