Palliative Care Nurse, RN Nurse, Clinical Nurse Specialist
Care Partners is seeking a compassionate and clinically skilled Registered Nurse Inpatient Hospice GIP to join our Healthcare & Medical Services team. In this role, you will provide high-quality, patient-centered hospice care to acutely ill patients receiving General Inpatient (GIP) hospice services within partnered inpatient facilities. Working closely with an interdisciplinary team—including physicians, social workers, chaplains, case managers, and facility staff—you will deliver comprehensive symptom management, pain control, and emotional support to patients and their families during end-of-life care.
As a Registered Nurse Inpatient Hospice GIP, you will perform thorough patient assessments, develop and implement individualized plans of care, administer medications and treatments, and continuously monitor patient status to ensure comfort and dignity. You will collaborate with providers across the Care Partners network to support coordinated, efficient care, serve as a clinical resource on hospice best practices, and communicate clearly and compassionately with families about goals of care, changes in condition, and available support services.
You will document all care accurately and timely in the electronic health record, adhere to regulatory and accreditation standards, and follow agency and facility policies related to infection control, patient safety, and quality improvement. This position requires strong critical thinking skills, the ability to respond quickly to changes in patient condition, and comfort with managing complex pain and symptom regimens in collaboration with hospice medical directors.

This is a full-time position with a primarily field/on-site work environment in inpatient partner facilities. Candidates should bring prior inpatient hospice, palliative care, or acute care experience, strong clinical assessment skills, and a deep commitment to dignified end-of-life care.
