Registered Nurse - Field Nurse - Home Health

We work with families and their loved one’s doctors and other health care providers to build a care plan that meets their needs.

Full‑time Registered Nurse (RN) – Wilmington, DE area. Day shift hours, one weekend a month, two holidays a year, and two on‑call shifts a month.

• Deliver skilled nursing care and prescribed treatment to geriatric patients in the community.
• Be IV proficient; will receive training and be expected to achieve proficiency within 90 days of hire.
• Develop and implement goal‑oriented, patient‑centered care plans consistent with available resources.
• Plan for discharge of patients.

• BSN required; ADN candidates must obtain a BSN within three (3) years of hire.
• Current Delaware RN license or active compact state license.
• One year of RN experience; prior home‑care and med‑surgical experience preferred.
• IV proficiency or willingness to train and perform IV functions.
