Guide Star Eldercare is seeking a Nurse Practitioner to address the needs of seniors with dementia in nursing homes and assisted living facilities. You will work autonomously with an assigned caseload while benefiting from corporate clinical leadership and centralized scheduling, coding, and billing support.
You will travel daily to facilities, conduct initial evaluations, ongoing therapy, and cognitive testing, and document service needs for each patient.
