Position Summary

UNC Horizons at Wake seeks a two‑year time‑limited Reentry Project Coordinator to provide clinical, supervisory, and programmatic leadership for a justice‑informed behavioral health initiative serving pregnant, parenting, and single women with substance use and co‑occurring health conditions. The position is responsible for identifying, recruiting, and enrolling a minimum of 80 participants into services annually; conducting comprehensive clinical assessments and diagnosing mental health conditions using the DSM‑5 manual;

and coordinating service linkage with behavioral health and community providers to ensure continuity of care. The Reentry Project Coordinator oversees project staff, provides training and clinical guidance, and supports interdisciplinary service delivery and ensures clinical services are adequately staffed. The role also leads data monitoring and evaluation activities, including surveys, outcome tracking, and reporting, and participates in biweekly evaluation meetings to identify program progress, gaps, and improvement strategies.

Additionally, the position strengthens partnerships with justice‑involved agencies and community stakeholders through ongoing outreach and representation at local, regional, and state events to enhance program visibility and impact.
