Pinnacle Family Services, part of Health Connect America, seeks a Family Centered Treatment (FCT) Specialist to deliver evidence-based services in homes and communities.
You will guide families through the four FCT phases, develop Service Plans, coordinate with medical, school, and community partners, and document care in the EHR.
Qualifications:
Bachelor’s in Human Services, 2+ years with youth or families, valid driver’s license; FCT certification is a plus and we’ll help you obtain it.
