Job Title: Family Reentry Support and Help (FRESH) Care Manager

Location: Wadesboro, NC (Anson County, NC - Polkton, NC)

Hours: 40 hours/week, Full Time, M-F, Exempt

Coastal Horizons Center, Inc. is seeking a qualified Care Management professional for our Family Reentry Support and Help (FRESH) program. This is a hybrid position that works at the Anson Women’s Correctional Facility and from home.

Provide case management and related services focused on the child-parent relationship and the re-entry/transition process for incarcerated parents and their children, both within correctional settings and in the community.

• Education:
  
  Bachelor’s degree in human services or related field. Master’s degree in social work or related field preferred.
• Certifications:
  
  Preference to individuals with North Carolina Peer Support Specialist certification and minimum 2 years of experience, or registered with the North Carolina Addiction Specialist Professional Practice Board (NCASPPB).
• Experience:
  
  One year working with individuals, families, and children involved in a correctional setting and the community.
• Skills:
  
  Flexible work schedule, travel, complete required training and background check from NC Department of Adult Correction (NCDAC), complete virtual Training for Parenting Inside Out (PIO). Proficient computer skills (internet, word processing, spreadsheets, email). Bilingual Spanish/English speaking skills a plus.

Salary Range: $42,000 - $45,000 commensurate with experience, plus medical/dental insurance, life insurance, disability insurance, retirement savings plan/401K, paid time off programs.
