Job Posting

Provides therapeutic case management services for Coastal Horizons housing, homeless and residential levels of care. Duties include screening, admission, orientation for these programs, assessment of client’s life domain areas, linkage to services in the community, income assistance and employment, assistance coordination of service linkage, medical and medication needs in coordination with client medical needs.

Home visits, inspections, gathering, entering and documenting client information and interactions, and participation in local Continuum of Care meetings to enhance communication and continuity of care.

Case manager will also provide or arrange transportation for clients to and from treatment, medical appointments, and other community resource appointments, and monitor client compliance with program expectations.

Responsibilities
  • Screen, admit, and orient clients to Coastal Horizons programs.
  • Conduct comprehensive assessment of clients’ life domain areas.
  • Link clients to community services, income assistance, employment, and medical and medication needs.
  • Co‑ordinate service linkage and assist with medical and medication management.
  • Perform home visits and inspections.
  • Gather, enter, and document client information and interactions.
  • Participate in local Continuum of Care meetings.
  • Arrange transportation for clients to treatment, medical appointments, and community resources.
  • Monitor client compliance with program expectations.
  • Participate in admissions and discharges, and respond to emergency client needs.
  • Assist clients in accessing pharmaceutical care charity programs.
  • Develop and maintain a network of supportive agencies for referrals.
  • Communicate and collaborate with apartment owners, landlords, and housing agencies.
  • Engage in regular consultation with team members and supervisor for high‑quality services and documentation.
  • Coordinate with QRT, ORHP and Homeless Outreach Teams; attend weekly team meetings.
  • Conduct monthly case management sessions with participants to further goals of permanent supportive housing.
  • Transport clients utilizing a company car or van.
Required Educational/Experiential Qualifications
  • Bachelor’s Degree in human services.
  • Experience and skills in substance use and/or mental health case management.
  • Training to provide evidence‑based care coordination, clinical screens, and support treatment recommendation follow‑through.

OR

  • An equivalent combination of education and experience.
Position Highlights

  • Subject to environmental hazards and potential exposure to human body fluids; subject to OSHA requirements on blood‑borne pathogens.

  • Strong interpersonal communication and professional boundary skills.
  • Knowledge of community resources for clients with substance abuse problems.
  • Ability to represent the Agency in community forums and conduct advocacy efforts.
  • Knowledge of laws, regulations, and policies governing mental health and substance abuse treatment, residential services and supportive housing.
  • Effective oral and written communication.
  • Strong organization skills and schedule management.
  • Comfortable working with client populations.
  • Basic computer skills.
  • Ability to network and coordinate efforts between treatment, medical, and other support systems.
  • Physical ability to perform basic life operational functions (climbing, stooping, crouching, reaching, etc.).
