Position InformationThe CQI Associate - Integrated Behavioral Health at our Freedom Hill Community Health Center supports the patients and care teams in the Integrated Behavioral Health (IBH) department. The CQI Associate-IBH manages referral work queues for incoming and outgoing IBH related services, patient scheduling and reminder calls, grant tracking and reporting, logistical planning for IBH CIT meetings, and other duties. The position reports to the Director of Integrated Behavioral Health.

Key Responsibilities- Assists with conducting audits and utilizes software systems to generate reports using Microsoft Excel and other
databases, including operational monitors and other productivity reports. - Assists with care coordination efforts, including care gap closures, patient mailings, Care Messaging
campaigns and other activities as needed. - Assists with internal and external referral tracking and closure for psychiatry referrals.
- Performs general outreach, engagement, and follow-up with patients.
- Assists with IBH and MAT appointment scheduling and reminder and follow-up calls.
Minimum Qualifications
- High school diploma or GED.
- One year of experience working with individuals with behavioral health conditions.
- I/DD and MH/SUD, preferred.
- Must have the ability to communicate effectively with medical professionals, third party
vendors and patients, in writing, in person, and by telephone. - Proven computer skill required for the production of correspondence, forms, reports, and spreadsheets.
- Must be proficient in Microsoft Excel and Microsoft Word.
High school diploma or equivalent.
ScheduleMonday, Tuesday, Thursday, and Friday, 8 am - 5 pm
Wednesday 11am - 8pm (8 hour shift)
Day shift
- Constant standing and walking, occasional bending, stooping, lifting, reaching forward and overhead.
- May occasionally be required to lift patients, office equipment and paper, usually not exceeding fifty pounds of direct lift.

The starting pay for this position is $13.25 per hour. Please keep in mind that actual salary will vary based on various factors such as qualifications, skills, competencies, and proficiency for the role.
