Job Summary
Wilson Medical Center offers a $20,000 commitment incentive bonus. The schedule is 5x8 rotating weekends/holidays, and the position is in‑person.
About the UnitWe are a 90‑bed Skilled Nursing & Rehabilitation Unit: 20 rehab beds and 70 LTC beds, staffed with Registered Nurses (RNs), Certified Nursing Assistants (CNAs), and medication aides. Patient ratios are 1:15 days, 1:19 evenings, and 1:25 nights. Patients include those with CHF, COPD, diabetes, ESRD, renal failure, dementia, stroke, and heart disease. This role offers an opportunity to work collaboratively in a supportive environment focused on providing quality care and support to individuals requiring temporary rehab assistance and LTC needs.
Responsibilities- Accurately perform patient assessments and identify patient needs.
- Identify and initiate appropriate nursing interventions.
- Provide care appropriate to condition and age of the patient.
- Perform timely and appropriate documentation relating to medical necessity in the medical record.
- Responsible for completion and revision of the Interdisciplinary Care Plan for each patient.
- Perform timely and accurate QI assessments.

A compassionate RN with a strong foundation in holistic and compassionate long‑term care, detailed patient assessment, medication administration, and chronic disease management. Applicants must have a current state RN license and a degree from an accredited nursing school.
- Basic Life Support certification required within 30 days of hire.
- One year of experience preferred.
