Position: PRN Hospice RN Case Manager – Yamhill County

PRN Hospice Registered Nurse Case Manager (RNCM)

Yamhill County, Oregon

Forest Park Hospice is seeking a compassionate and dependable PRN Registered Nurse Case Manager (RNCM) to serve patients and families throughout the Yamhill County area.

This position will initially manage a small caseload of approximately five to six hospice patients, allowing the RNCM to provide attentive, individualized care while helping us expand our presence in the community. As the Yamhill County caseload grows, this position has the potential to develop into a full-time opportunity.

Position Responsibilities

  • Manage and coordinate hospice care for an initial caseload of approximately five to six patients.
  • Complete comprehensive nursing assessments and develop individualized plans of care.
  • Perform scheduled and as-needed nursing visits in patients' homes and care facilities.
  • Monitor patients for changes in condition, symptom burden, comfort, and safety.
  • Provide symptom-management education and emotional support to patients, families, and caregivers.
  • Communicate changes in condition promptly to the hospice provider and interdisciplinary team.
  • Coordinate medications, medical equipment, supplies, and other services related to the hospice plan of care.
  • Participate in interdisciplinary team meetings and contribute to care planning.
  • Maintain timely, accurate, and complete clinical documentation.
  • Support patients and families during periods of decline, crisis, and end-of-life transition.
  • Collaborate respectfully with facility staff, providers, pharmacies, and community partners.
  • Participate in the on-call rotation or provide coverage as assigned based on agency needs.

Qualifications

  • Current and unrestricted Oregon Registered Nurse license.
  • Current CPR certification.
  • Valid driver's license, reliable transportation, and current automobile insurance.
  • Ability to travel throughout Yamhill County.
  • Strong nursing assessment, communication, organization, and time-management skills.
  • Ability to work independently while remaining closely connected with the interdisciplinary team.
  • Hospice, palliative care, home health, oncology, geriatrics, or case-management experience is preferred.
  • Comfort providing education and support to patients and families facing serious illness and end-of-life care.
  • Familiarity with electronic medical records is preferred.

Schedule and Growth Opportunity

This is a PRN position beginning with an established caseload of approximately five to six patients. The schedule will be based on patient needs, required visit frequency, and coordination with the clinical team.

As referrals and the Yamhill County census increase, the RNCM will have the opportunity to take on additional patients and may transition into a full-time position.
