RN Case Manager: Home Health & Hospice

Seeking compassionate, skilled Registered Nurses to join our Home Health and Hospice teams. In this role, you will provide hands‑on clinical care, coordinate individualized care plans, and build trusted relationships with patients and families throughout Litchfield County and the Greater Farmington Valley.

• Full-time, Monday-Friday schedule
• Weekend rotation: one every six weeks.

• Perform comprehensive nursing assessments.
• Develop and manage individualized care plans.
• Provide skilled nursing care in patients’ homes.
• Educate patients and caregivers.
• Collaborate with physicians and interdisciplinary team members.
• Document patient care accurately and efficiently.
• Respond to changing patient needs with clinical judgment and compassion.

• Active Connecticut RN license
• Current CPR certification
• Valid driver’s license and reliable transportation
• Strong communication and assessment skills
• Minimum of one year of home care and/or hospice experience preferred.
