CenterPoint Health is seeking a compassionate and skilled Registered Nurse (RN) to join our Healthcare & Medical Services team, delivering integrated, community-focused care to patients across primary care, dental, and behavioral health services. As an RN with CenterPoint Health, you will play a key role in improving the health of underserved individuals and families by providing high-quality, patient-centered nursing care in a collaborative, interdisciplinary environment.
In this role, you will conduct comprehensive patient assessments, obtain health histories, and perform focused physical exams to support accurate diagnosis and treatment. You will administer medications, vaccinations, and treatments according to provider orders and established protocols, while closely monitoring patient responses and documenting all care in our electronic health record system. You will coordinate care plans for patients with acute and chronic conditions, collaborate with physicians, advanced practice providers, behavioral health professionals, dental providers, and other members of the care team, and help ensure timely follow-up and continuity of care.
A core part of your work will involve patient and family education. You will provide clear, culturally sensitive teaching on disease management, medications, lifestyle changes, preventive care, and available community resources to empower patients to manage their health. You will support patients with chronic diseases such as diabetes, hypertension, and asthma, assist with care transitions, and help address behavioral health needs through screening, referral, and close coordination with our behavioral health staff.
The RN will also participate in triage, both in person and by phone or telehealth, helping determine patient acuity and appropriate care settings. You will obtain and interpret vital signs, perform point-of-care testing, assist with procedures, and ensure adherence to infection control and safety standards. You will contribute to quality improvement initiatives, population health efforts, and care management programs aimed at reducing health disparities and improving measurable outcomes for our community.
