The Winchester Transition Specialist is a Winchester High School‑based Specialist who serves the district to develop and provide direct services and training to both students and staff to facilitate post‑secondary options for students with disabilities as they prepare to transition to the high school, out of the high school, and onto post‑graduate programming, employment opportunities, and/or colleges/universities/other institutions.
Essential Duties and Responsibilities- Complete transitional and vocational assessments on students as assigned.
- Summarize and present results of assessments at IEP Team meetings and make appropriate recommendations in the development of the IEPs, including the development of measurable related goals and benchmarks, and the direct implementation of the IEP, including but not limited to the service delivery.
- Collaborate with classroom teachers and staff in the implementation of IEP services, goals, and objectives related to transition.
- Provide direct service to the student, relative to transition, prevocational, vocational, and other related goals and objectives.
- Develop community employment‑focused learning opportunities/internships based on identified student interests. Maintain a record, including dates, times, and outcomes, of all related efforts to secure career exploration opportunities, community‑based training, recreational activities, and work‑based learning opportunities.
- Provide access to vocational/educational/recreational opportunities within the school and community settings.
- Develop and provide career awareness, readiness, and exploration opportunities (job shadows, internships, informational interviews) directly to individual students as needed.
- Directly teach and train students in appropriate work‑related attitudes, self‑advocacy skill building, safety, habits, behavior, dress, travel options/skills, and social/communication skills to prepare them for post‑secondary employment and community settings.
- Place students in appropriate vocational/educational/recreational settings, with job coach support as needed, and provide direct supervision and assessment of the students' functioning in the placement and activities.
- Monitor the need for and coordination of job coaches.
- Provide direct training to individuals serving as job coaches with employers regarding student progress, strategies for improvement, and worksite accommodations.
- Ability to run team meetings/re‑evaluation meetings when the program supervisor is not available.
- Identify transition services provided by community agencies and share information with appropriate individuals.
- Facilitate individual referrals to other agencies as needed.
- Collaborate with guidance counselors to assist identified students to examine/identify post‑secondary training and education options connected to career interests.
- Coordinate and collaborate on a yearly Transition Fair.
- File Chapter 688 Referrals with the appropriate state agency (MRC, DDS, DMH, etc.) and assist students and families with accessing support from the correct agency.
- Compile, write, maintain, and file all reports, records, and other documents required, including Transition Planning Form, Student Summaries, and other identified documents as assigned by the supervisor.
- Perform other duties as assigned by the Supervisor, Building Principal, and/or Special Education Director.
Compensation Range: $63,787—$136,191
