Licensed Practical Nurse (LPN) | Winchester Rehabilitation and Nursing Center Job Summary

We are seeking a compassionate and dependable Licensed Practical Nurse (LPN) to join our skilled nursing team at Winchester Rehabilitation and Nursing Center in Winchester, MA. As an LPN at our 117-bed skilled nursing facility, you will provide direct nursing care, supervise nursing support staff, and ensure high-quality, resident-centered care in accordance with facility policies and regulatory requirements.

Responsibilities
  • Supervise Certified Nursing Assistant (CNA) staff and other nursing personnel on assigned units
  • Provide direct care, including medications, wound care, colostomy changes, tube feedings, suctioning, IV administration, and other skilled nursing interventions as assigned
  • Observe, document, and report changes in residents’ status to physicians, families, and leadership
  • Transcribe and carry out the physician's orders accurately
  • Participate in admissions, discharges, and transfers
  • Conduct rounds to ensure resident needs are met, and staff follow facility policies
  • Schedule follow-up appointments and coordinate transportation needs for residents
  • Collaborate with the interdisciplinary care team to ensure holistic care
  • Maintain adequate stock of medications, supplies, and equipment; notify appropriate personnel of needs
  • Promote teamwork, mutual respect, and effective communication
  • Participate in survey processes, inspections, and QAPI activities
  • Treat all residents with dignity and respect, protecting residents’ rights and maintaining confidentiality
  • Follow infection control policies, safety procedures, and life safety/emergency protocols
  • Report any abuse, neglect, exposure incidents, or noncompliance according to policy
  • Complete all assigned training and skills competency requirements
Qualifications
  • Current, unencumbered Massachusetts LPN license
  • Strong communication and critical-thinking skills
  • Ability to handle situations with tact and professionalism
  • Prior long-term care experience preferred but not required
