We are seeking a compassionate and reliable Home Hospice Aide to provide personal care and supportive services to clients in their homes across Virginia.

• Follow individualized care plans under the supervision of a Registered Nurse
• Assist clients with personal care and hygiene needs
• Help maintain a clean, safe, and healthy environment in the client’s home
• Support with ambulation and prescribed exercise programs
• Prepare meals and assist with grocery shopping, as directed
• Provide companionship and promote mental engagement through activities
• Offer emotional support to clients and their families
• Monitor and report any changes in the client’s condition
• Document visits and services provided in a timely and accurate manner
• Participate in in‑service training sessions
• Take and record vital signs if specified in the care plan
• Assist with medications that are ordinarily self‑administered, as allowed under Virginia law

• Certified Nurse Aides (CNA)
• Home Health / Home Hospice experience preferred

• Reliable transportation and the ability to access clients’ homes (some homes may not be wheelchair accessible)
• Physical ability to assist clients, including lifting up to 30 pounds and supporting client weight up to 250 pounds
• Must be able to perform tasks requiring hearing, vision, and manual dexterity
• Must maintain client confidentiality and adhere to all company policy
