** Job Description
** The Community Health & Wellness Services Coordinator provides administrative and operational support to the Senior Director of Community Health and the Manager of Wellness Services. This position is responsible for coordinating day‑to‑day departmental activities, supporting clinical and wellness program operations, and serving as a primary liaison for internal staff, patients, and program participants. Core responsibilities include coordinating medical nutrition therapy (MNT) appointments, managing referral tracking systems, processing Healthy U wellness program files, maintaining screening and eligibility records, and supporting the health system’s community benefit reporting efforts.
The Coordinator also manages departmental scheduling, meeting logistics, supply ordering, and general office functions to ensure smooth and efficient workflow.
Responsibilities and Duties Schedule MNT appointments, register patients in EPIC, and manage patient check‑in processes.
Maintain referral tracking spreadsheets and reporting mechanisms for all MNT referrals and follow‑up.
Communicate with patients regarding appointments, questions, and scheduling needs.
Collaborate with referring practices to coordinate patient care.
Serve as the primary point of contact for Healthy U participants.
Process monthly Healthy U wellness incentive files and eligibility data.
Upload, track, and maintain employee screening and wellness participation records.
Coordinate with Human Resources, IT, and external wellness vendors to support program administration.
Track and maintain community benefit data for the entire health system.
Utilize community benefit tracking software to record, audit, and analyze activities.
Work closely with a team of community benefit “super users” to ensure accurate reporting.
Interface with community partners as required and occasionally attend community events on behalf of the department.
Coordinate and schedule meetings for Community Health & Wellness Services leadership.
Serve as the scheduler for Wellness Center Conference Rooms.
Act as department timekeeper, ensuring accurate time reporting and corrections.
Manage office operations including ordering supplies, answering phones, and responding to public inquiries about services.
Assist with general administrative tasks to support smooth departmental workflow.
** Education
* * High school diploma or GED required.
Associate’s degree in a related field (e.g., Health Administration, Business Administration, Public Health) preferred.
** Experience
* * 1–2 years of administrative experience in an office, customer service, or healthcare setting.
Experience with Microsoft Office Suite (Excel, Outlook, Word) and comfort working with spreadsheets and tracking tools.
Customer service experience, especially involving direct interaction with the public, patients, or employees.
** Qualifications
* * Ability to maintain confidentiality of patient and employee information in accordance with HIPAA and organizational policy.
Ability to work independently with limited supervision while also collaborating effectively with multiple departments.
Ability to build positive relationships with referring practices, wellness vendors, community partners, and health system staff.
Ability to adapt to changing priorities, new technologies, and evolving program needs.
Ability to represent the department professionally at community events, within the health system, and when engaging with the public.
Ability to apply sound judgment, escalate issues appropriately, and solve routine problems with minimal guidance.
Strong organizational and time‑management skills, with the ability to prioritize multiple tasks and manage competing deadlines.
