This is a full‑time, on‑site Clinical Liaison role based in Winchester, VA. The Clinical Liaison will play a critical role in the organization by acting as a bridge between the hospital and patients. Responsibilities include evaluating patients for rehabilitation services, working closely with interdisciplinary teams to develop patient care plans, and advocating for patients’ medical and rehabilitation needs. Additionally, the Clinical Liaison will maintain relationships with referral sources and contribute to business development efforts to expand outreach and enhance continuity of care.

• Case Management expertise to assess and coordinate patient care and transition plans
• Familiarity with Home Care and Rehabilitation services to facilitate effective patient support
• Strong skills in Sales and Business Development to engage and nurture relationships with referral sources
• Excellent organizational, interpersonal, and communication skills
• Ability to work collaboratively in a medical and rehabilitation team environment
• Experience in healthcare or rehabilitation settings is highly valued
• Current Registered Nurse (RN), Physical Therapist (PT), Occupational Therapist (OT), Speech Therapist (SLP).
