Job Description
The LPN Case Manager will assist the RN Case Managers with their daily role and processes to organize and facilitate thorough discharge plans for patients. The LPN Case Manager will complete clinical assessments for patients requiring post-acute services under State Medicaid programs, including Virginia, West Virginia, and Maryland. Responsibilities also include completing pre-screens, admissions, and insurance authorizations for Valley Health’s skilled nursing facility and transitional care beds.
ExtraDuties
- Assisting Case Management staff with Level 2 (MIMR) assessment.
- Interviewing patients, family members, and/or caretakers as needed to complete assessments.
- Maintaining open communication with the care management team about approval/denial of clinical assessments.
- Maintaining proficiency in electronic submission programs and awareness of current policies and trends related to clinical assessments for Medicaid services.
- Assisting with the arrangement and follow-up with community resources, and facilitating patients’ transitions to the next level of care.
Graduate of an accredited school of nursing required.
ExperienceTwo years of bedside experience required.
Certification & LicensureLicensed Practical Nurse in the State of Virginia required. BLS Certification (Basic Life Support). American Heart ‘Healthcare Provider’ (HCP) certification is required; new hires must have AHA‑approved certification prior to orientation.
QualificationsCompetent to interact successfully with patients, addressing their special needs which may vary by age.
