Position: Coordinator, Patient Disease - BSN Required

CDRC Patient Disease Coordinator is responsible for coordinating inpatient/outpatient education for patients with Chronic Disease.

Responsibilities
  • Develop and apply evidence‑based nursing practice standards to achieve optimal patient outcomes.
  • Oversee evidence‑based, individualized, effective care for patients and families in an environment of clinical excellence.
  • Assist in maintaining a standardized patient education program regarding disease, symptoms, and medications.
  • Assist in the development of a standardized plan of care with measurable goals for patients with chronic disease.
  • Initiate contact with patients in the acute care setting to provide initial/basic education and encourage follow‑up post‑discharge.
  • Serve as an educational resource to hospital staff, patients, families, and outpatient primary‑care physician staff.
  • Build relationships with community resources to connect patients with services.
  • Implement and maintain education classes and coordinate content experts to deliver education.
  • Maintain a disease‑specific patient registry.
Qualifications
  • Bachelor of Nursing required;
    Master of Nursing preferred.
  • Virginia RN licensure required; ability to obtain licensure in West Virginia required.
  • BLS Certification (Basic Life Support) – American Heart Association approved required.
  • Telehealth certification required.
  • Knowledge and skills to develop adult education programs required.
  • New hires must possess AHA appropriate certification prior to completion of orientation.
