CDRC Patient Disease Coordinator is responsible for coordinating inpatient/outpatient education for patients with Chronic Disease.
Responsibilities- Develop and apply evidence‑based nursing practice standards to achieve optimal patient outcomes.
- Oversee evidence‑based, individualized, effective care for patients and families in an environment of clinical excellence.
- Assist in maintaining a standardized patient education program regarding disease, symptoms, and medications.
- Assist in the development of a standardized plan of care with measurable goals for patients with chronic disease.
- Initiate contact with patients in the acute care setting to provide initial/basic education and encourage follow‑up post‑discharge.
- Serve as an educational resource to hospital staff, patients, families, and outpatient primary‑care physician staff.
- Build relationships with community resources to connect patients with services.
- Implement and maintain education classes and coordinate content experts to deliver education.
- Maintain a disease‑specific patient registry.
- Bachelor of Nursing required;
Master of Nursing preferred. - Virginia RN licensure required; ability to obtain licensure in West Virginia required.
- BLS Certification (Basic Life Support) – American Heart Association approved required.
- Telehealth certification required.
- Knowledge and skills to develop adult education programs required.
- New hires must possess AHA appropriate certification prior to completion of orientation.
