Registered Nurse (RN) Case Manager - Hospice

We are seeking a dedicated and compassionate Registered Nurse (RN) Case Manager - Hospice to provide high-quality, patient-centered care to individuals with terminal illnesses.

This role offers a meaningful opportunity to support patients and their families during end-of-life care while working in a collaborative and supportive healthcare environment.

As an RN Case Manager, you will be responsible for coordinating and delivering hospice care services across home and facility settings, ensuring comfort, dignity, and quality of life for each patient.


Position Summary

The RN Case Manager - Hospice is responsible for overseeing and coordinating the care of hospice patients, including clinical assessments, care planning, and ongoing case management. This role requires strong clinical judgment, compassionate communication, and the ability to work independently while collaborating with an interdisciplinary team.

Key Responsibilities
  • Provide direct nursing care, comfort, and emotional support to hospice patients
  • Conduct comprehensive patient assessments and develop individualized care plans
  • Manage a caseload of approximately 10-12 patients, completing ~5 visits per day
  • Administer medications, treatments, and interventions as appropriate
  • Monitor patient condition and adjust care plans based on clinical needs
  • Coordinate care with physicians, social workers, aides, and other team members
  • Educate patients and families about disease progression, care plans, and end-of-life support
  • Document patient care accurately using Matrix Care EMR
  • Participate in on-call rotation (Monday-Thursday, every other week) and respond to urgent patient needs when necessary
  • Ensure compliance with all regulatory, safety, and hospice care standards
Minimum Requirements
  • Active Registered Nurse (RN) license in the practicing state
  • Current CPR/BLS certification
  • TB screening (PPD, questionnaire, or chest X-ray as required)
  • Current health certificate (as required by state or contract)
  • Compliance with all federal, state, and local employment requirements
  • Must be at least 18 years of age
Preferred Qualifications
  • Prior hospice, home health, or case management experience
  • Strong clinical assessment and care coordination skills
  • Ability to work independently in a field-based setting
  • Excellent communication, empathy, and interpersonal skills
  • Experience with EMR systems (Matrix Care preferred)
Assignment Details
  • Setting: Hospice (Home & Facility Visits)
  • Schedule: Monday - Friday, 8:00 AM - 4:30 PM
  • Hours: 40 hours per week
  • Contract Length: 10 weeks (with potential extension)
  • Start Date: June 22, 2026
  • Territory: Shenandoah & Warren County (condensed daily routes)
  • On-Call: Monday-Thursday, every other week (primarily phone triage with occasional visits)
  • Mileage Reimbursement: $0.70 per mile
Compensation
  • Local RN: ~$2,000/week ($50/hr)
  • Travel RN: ~$2,366/week (includes stipends)
  • Weekly pay available
