About the Role

We’re seeking a dedicated Registered Nurse (RN) to join our PACE interdisciplinary team in a primarily field-based, in-home nursing role. In this position, you will provide direct clinical care to participants in their homes, assisted living facilities, and community settings, supporting older adults in maintaining safety, independence, and dignity within their preferred living environment.

This role is ideal for a nurse who values autonomy, strong clinical assessment skills, and meaningful relationships with participants and families.

How You’ll Make a Difference
  • Conduct comprehensive in-home nursing assessments and ongoing evaluations of participant status.
  • Perform skilled nursing visits including medication management, wound care, injections, phlebotomy, and chronic disease monitoring.
  • Assess changes in condition and communicate promptly with providers and the interdisciplinary team (IDT).
  • Develop, implement, and monitor individualized care plans in collaboration with the IDT.
  • Provide participant and caregiver education on disease management, medications, safety, and prevention strategies.
  • Coordinate care transitions following hospitalizations, ER visits, or facility stays.
  • Support participants residing in ALF or other community settings as needed.
  • Identify safety concerns in the home and collaborate with PT/OT and social work to address environmental risks.
  • Participate in interdisciplinary team meetings and contribute to holistic care planning.
  • Document all nursing care accurately and timely in the electronic medical record.
  • Respond to urgent clinical needs and participate in on-call rotation as assigned.
  • Monitor medication adherence and assist with medication setup or reconciliation.
  • Collaborate with external providers, pharmacies, home health agencies, and specialty services.
  • Perform other related duties as assigned.
What You Bring
  • Current Registered Nurse (RN) licensure in the Commonwealth of Virginia.
  • Current BLS/CPR certification.
  • Minimum of one year of experience working with the frail or elderly population.
  • Strong clinical assessment and critical thinking skills.
  • Experience with home health, geriatrics, hospice, or community-based care preferred.
  • Medical clearance for communicable diseases and up-to-date immunizations prior to participant contact.
  • Valid driver’s license, proof of insurance, and reliable transportation (required due to field-based nature of the role).
  • Ability to work independently while maintaining strong communication with a collaborative team.
