POSITION TITLE: Licensed Practical Nurse

DEPARTMENT: Nursing

REPORTS TO: Director of Nursing

POSITION SUMMARY: Full time, part time, PRN

$1,500 SIGN ON BONUS

Qualifications
  • Must possess a valid LPN license, in good standing in accordance with state laws, from an approved nursing school.
  • Must possess at least a high school diploma or GED.
  • Must be able to read, write, speak, and understand English.
  • Must maintain the highest standards in caring for residents and their families.
  • Must adhere to Viva Senior Living’s attendance policies and follow employment terms as outlined in the Employee Handbook.
  • Must comply with federal, state, and local health and regulatory standards and guidelines.
  • Must collaborate with peers in a congenial work environment and maintain compassion and dignity toward residents, adhering to the Resident Bill of Rights.
  • May be required to respond to call backs in emergency conditions such as severe weather, evacuation, or staffing shortages.
Responsibilities / Accountabilities
  • Coordinate the day‑to‑day care of residents according to individual service plans, ensuring sufficient staffing, supervising RMAs and direct‑care staff.
  • Provide knowledge of individualized care plans and support residents as directed, contributing to care planning with observations for the charge nurse or other care‑planning staff.
  • Complete state‑required assessment and service‑plan training.
  • Coordinate hospice and home‑health services, reviewing outside provider communications and participating in wound monitoring/management per policies.
  • Coordinate durable medical equipment services to ensure residents receive required equipment.
  • Coordinate medication management with pharmacies, managing medication orders, ensuring medication availability, handling medication destruction, managing medication carts, and auditing medication services.
  • Administer and document medication and treatments per physicians’ orders, accurately recording all care provided.
  • Report care provided and changes in resident condition to responsible parties, primary care providers, and Resident Care Director.
  • Review end‑of‑shift reports, Med Tech logs, and respond accordingly.
  • Respond to resident emergencies with appropriate action and follow‑through.
  • Document incident reports as required.
  • Conduct community audits and training to ensure compliance with state regulations and company policies.
  • Participate in routine resident care activities, including daily living and basic nursing procedures.
  • Respond to inquiries from residents, visitors, and staff promptly and courteously per policy.
  • Communicate and interact effectively with residents, visitors, families, and peers.
  • Arrive to work at scheduled time.
  • Perform other related duties as required.
  • Ensure residents and families receive the highest quality service in a caring and compassionate atmosphere.
  • Report occupational exposures to blood, body fluids, infectious materials, hazardous chemicals; report injuries and illnesses to supervisor; complete all assigned training and skill competency; participate in life‑safety and emergency drills.
  • Assist with evacuation during emergencies and maintain confidentiality of Protected Health Information.
  • Report policy non‑compliance, abuse, neglect, misappropriation, retaliation, discrimination, and possible crimes to the appropriate parties.
Annual Health Requirements
  • Provide annual verification of a negative TB skin test.
Physical and Sensory Requirements
  • Move intermittently throughout the workday.
  • Cope with mental and emotional stress of the position.
  • See and hear or use prosthetics to ensure senses function adequately.
  • Meet general health requirements, including medical and physical examination.
  • Relate to ill, disabled, elderly, emotionally upset, or agitated residents.
  • Push, pull, move, or lift a minimum of 25 pounds to a height of 5 feet and a distance of 50 feet.
  • Assist in evacuation of residents during emergencies if necessary.
