About the Role
As an RN Case Manager, you'll provide direct patient care, coordinate individualized care plans, and serve as a trusted presence for patients and families. You'll collaborate with an interdisciplinary team to ensure every patient is cared for - body, mind, and spirit. With a strong focus on clinical judgment, communication, and organizational skills, you'll be responsible for:
Assessing patient and family needs and developing personalized care plans
Delivering skilled, compassionate nursing care and symptom management
Administering medications and coordinating care across the healthcare team
Educating and supporting families to promote comfort and peace of mind
Leading and guiding hospice aides to ensure consistent, compassionate care
Maintaining accurate documentation and participating in ongoing education
What You'll Bring
Current RN license in Virginia (multi-state or single-state)
CPR certification and at least two years of nursing experience (hospice or home health preferred)
Strong clinical judgment, communication, and organizational skills
Reliable transportation, valid driver's license, and insurance
