Job Summary

The THRIVE Case Manager provides a holistic approach to case management, aiding in the facilitation of employment, career plans, systemic referral procedures, and housing stability. Acting as a "Hub" for the THRIVE collective, the manager ensures participants do not fall through the cracks of the referral system and provides accountability to each participant. Responsibilities include maintaining an active caseload, performing data entry, and preparing and submitting mandatory reports.

Functions
  • Participate in the development and implementation of case‑finding and participant‑recruitment initiatives.
  • Perform screening, intake, stabilization plans, and orientation functions with participants in Winchester, Frederick, and Clarke County to facilitate entrance into appropriate programs and services.
  • Act as case manager, providing monthly case‑management services, including information and referrals to THRIVE program resources and additional community services.
  • Implement training and interventions to advance participants’ career development, including Get Onboard!, job‑seeking support, employment retention support, and barrier resolution.
  • Serve as liaison between participants, employers, landlords/property managers, referral agencies, and funding sources.
  • Provide post‑employment support and help participants identify and utilize natural support systems.
  • Educate participants and community organizations about the THRIVE program and services.
  • Advocate for and hold participants accountable.
  • Maintain strong rapport with participants, employers, landlords/property managers, THRIVE partners, and referring agencies.
  • Maintain participant records, case notes, attendance records, and progress reports.
  • Perform mandatory data entry and outcome tracking; complete and submit mandatory reports and program proposals to funding sources.
  • Develop and practice participant retention monitoring to reduce disengagement and promote long‑term stabilization.
  • Complete progress reports within the THRIVE program and submit fee‑for‑service invoices as directed.
  • Assist with program evaluation and community‑needs assessment to identify program and service needs.
  • Participate in the development and submission of program and service proposals to expand and diversify funding.
  • Participate in agency meetings and committees as directed.
Qualifications
  • Associate’s degree or high school diploma and two years of relevant experience required; bachelor’s degree preferred.
  • Personal experience with the public assistance system preferred.
  • Must possess a clean driving record and provide own transportation.
