Registered Nurse Case Manager – Full Time (Days)

• Annual salary: $77,000 – $96,000
• Mileage reimbursement

• Perform patient assessments and collaborate with the care team to develop and implement a plan of care
• Make referrals to other disciplines as indicated by the patient’s identified needs or document rationale for not doing so
• Promote patient health and independence through teaching and appropriate rehabilitative measures, assisting patients in learning self‑care techniques
• Supervise LPNs and HHAs
• Complete documentation timely and accurately
• Communicate patient progress regularly to the clinical manager and care team
• Plan and provide staff education
• Perform on‑call responsibilities and on‑call services to patients/families as assigned
• Participate in clinical development and continuing education programs
• Other duties as assigned

• Minimum one year of clinical experience as a registered nurse (RN); if less than 1 year, candidate must be approved by VP Clinical
• Current RN license specific to the state(s) assigned
• Current CPR certification
• Valid driver’s license, reliable transportation, and liability insurance
• Note:
  
  If less than 6 months of clinical experience as an RN, candidate must participate in the RN Intern program
